The original dev, Edmumd McMillen, wanted to add even more content, but due to the limitations of Flash, he couldn't. Eventually they released a couple DLCs for the game, which added bosses and lots of other items. However, the original was programmed in Flash, thus, had a ton of limitations. If you're unfamiliar with the original Binding of Issac, it's a rogue-like with lots of items and combos that work together.
